proof of enrolment in the 5th or 6th year of study
academic transcript for all completed years (1st–4th/5th year)
a professor’s recommendation letter (exclusively for the purpose of this scholarship application)
a signed statement confirming that you are not receiving any other scholarship (or a statement of withdrawal from another scholarship)
proof of demonstratorship (if applicable)
confirmation of application for the Dean’s/Rector’s Award (if applicable)

For dental technician and dental assistant pupils:

proof of enrolment in the 4th year of secondary school
academic transcript for the 1st to 3rd year
a signed statement confirming that you are not receiving any other scholarship (or a statement of withdrawal from another scholarship)
